Have your say on health care in your local community!

Express your interest in volunteering for our Consumer Health Panel - its your chance to be the voice for your local community! You will be asked to provide feedback on a range of health care issues or services that impact upon you, your family or friends.

Whether its access to after hours health services, your ability to see a doctor when you need to, or the new My Health Record - we want to know your thoughts on what's working and what isn't!

By sharing your views, you can help shape the future of our health care system!

Who are we looking for?

We're looking for residents from South Eastern NSW, aged 18 years or older, and a regular user of health care services. Sorry but people who work in the health sector are not eligible.

Expression of interest form

Please complete the on-line application form. The information provided will only be used to ensure that a diverse range of consumers are represented on our Consumer Health Panel - after all we want a broad representation of our community. COORDINARE commenced operations as the South Eastern NSW Primary Health Network (PHN) in July 2015 and is one of 31 PHNs established across Australia. PHNs were established by the Department of Health on 1 July 2015 with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.
